Advance Directives: Learn, Plan & Prepare

1:00 p.m. - 2:00 p.m. presentation 

 Join us for a two-part program on Advance Directives. Ellen Taylor, Dementia Care Specialist at the Aging & Disability Resource Center of Dane County, will discuss how to communicate what matters most to you, ensure your health care wishes are known, and explore advance care planning options.

 

2:00 p.m. - 3:00 p.m. optional workshop 

The second hour will include a hands-on workshop to ask questions and complete Wisconsin state forms. Forms, on-site witnesses, and a Notary will be provided.
